1. Introduction
These Terms and Conditions ("Terms") form a binding agreement between you ("client") and [Your Name/Practice Name] ("we", "us", "our") for the delivery of psychological, counselling, or mental health services in Queensland, Australia.
By attending or scheduling a session, you confirm your acceptance of these Terms.
2. Scope of Services
We provide individualised therapy, recovery coaching, drug and alcohol relapse prevention planning, personal development and psycho-education delivered by appropriately qualified and insured professionals. Services may be offered:
In person at a location of your choosing
Online via secure telehealth platforms
All clinicians are members of relevant professional bodies (e.g., AASW, AHPRA, ACA, APS, PACFA) and practice within their ethical codes.
3. Eligibility for Services
Clients must:
Be 18 years or older, or have a parent/guardian consent if under 18
Be capable of providing informed consent
Not be in immediate crisis (emergency situations should be referred to 000 or a crisis service)
4. Confidentiality and Privacy
Your privacy is protected under the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Confidentiality will only be breached if:
There is a serious and imminent risk of harm to yourself or others
There is a legal obligation to report (e.g., child abuse, court orders)
Information is shared with a supervisor for professional development (fully de-identified)
You provide written consent
We comply with the Health Records and Information Privacy Act 2002 (Qld) for managing health information.
5. Fees and Payments
Standard Fee: $120 per 50-minute session
Payment due after each session by cash, EFTPOS or bank transfer
Sliding scale or bulk billing may be available on a case-by-case basis
Missed payments may result in service suspension
6. Cancellation and No-Show Policy
Minimum 24 hours’ notice is required for cancellations
Late cancellations or missed appointments may incur the full session fee
Exceptions may apply in cases of emergency or illness
7. Telehealth Services
We offer telehealth via our secure platform, Zanda, which complies with Australian privacy and health laws. Clients must:
Be located in Australia during sessions
Ensure a private, quiet space
Have a stable internet connection
If technical issues prevent session completion, we will reschedule at no extra cost.
8. Informed Consent
By engaging our services, you acknowledge:
You understand the nature and purpose of therapy
You are aware of potential emotional discomfort that may arise during therapy
You can withdraw consent and discontinue at any time
Therapy outcomes are not guaranteed
9. Clinical Records
We maintain secure records of your sessions, stored in compliance with the Health Records Act (Qld) and retained for at least 7 years from the date of last contact (or until a minor turns 25).
10. Termination of Services
We may cease therapy if:
You consistently miss or cancel appointments without notice
There is a breakdown in the therapeutic relationship
Your needs are outside our scope of practice
You violate these Terms
We will provide referral options where possible.

12. Complaints and Feedback

13. Changes to Terms
